Medical Referrals
Under the current New York State Practice Act for Physical Therapists, a written referral from a qualified medical professional is required before treatment can commence.

How To Schedule An Appointment

• All patients must be referred by your medical
  doctor, podiatrist, or dentist.
• All HMO patients must have a referral number prior to
  scheduling your first appointment.
• Please arrive 15 minutes early for your first scheduled
  appointment, and be prepared to complete a Medical
  questionnaire.
• We request notification for all cancellations at least 24
  hours in advance.
• Allow 45 minutes to 1 hour to complete evaluation and
  treatment.
